What does the radiator cap actually do?

The cap has two valves. The main spring holds pressure in the system until it reaches the rated value, then lets excess coolant into the overflow. A small vacuum valve lets that coolant flow back as the engine cools. If either valve sticks, the level goes wrong and the engine runs closer to boiling than it should.

The rated value is printed right on the cap. Passenger vehicle caps commonly fall in the range of about 13 to 16 psi, and that pressure matters because every additional psi raises the boiling point of the coolant by roughly 3 degrees F. Those ranges are general; the rating your vehicle maker specifies is the one a replacement cap has to match.
